Enigmes médicales : on a rencontré la médecin qui a inspiré "Dr House"
Fri 28 Sep 2018 11:40:59 AM CEST - permalink -Il faut avoir en tête que tout ce qui arrive à un patient n'est pas essentiel à notre diagnostic : il faut comprendre et distinguer ce qui est important de ce qui arrive dans la vie et la santé d'une personne. Il existe deux aphorismes sur lesquels on s'appuie. Le "rasoir d'Ockham" , principe selon lequel l'idée la plus simple est souvent la bonne. C'est l'idée que l'on peut peut-être expliquer un ensemble de symptômes par une seule cause. C'est ce qu'on préfère !
Il y a aussi celui d'un docteur américain, qui a donné le "Hickam's dictum" [...] "Un patient peut avoir autant de satanées maladies qui lui chante." On espère que le patient n'a qu'une maladie mais il arrive qu'il en ait plusieurs...

Car s'il existe des milliers de maladies, notre corps a un nombre très, très limité de façons de les exprimer.

Personne ne le sait vraiment, mais je crois que c’est entre autres parce que les femmes ont été éduquées à écouter. C'est une compétence importante lors du diagnostic : recueillir l'histoire du patient.
Alors on est bien d'accord que les femmes n'écoutent pas "mieux" que les hommes, mais l'essentiel est dit : on n'apprend pas (assez) aux médecins à écouter leurs patients (pas en tant qu'être humain en tout cas), juste à les considérer comme un sac de symptômes.
C'est une étude faite dans les années 70 en Grande-Bretagne. Quatre docteurs ont reçu un total de 80 patients. Ils devaient garder une trace de ce sur quoi ils se sont fondés pour établir leur diagnostic. Dans 75% ou 80% des cas, le diagnostic a été fait à partir de l'histoire du patient. Les examens n'ont suffi qu'à obtenir des compléments d'information ou à confirmer ce qu'ils savaient déjà. Cette étude a été refaite dans les années 1990 et des résultats similaires ont été obtenus.
